Program Overview

The CCSD Transition Academy serves young adults, ages 18-22 in a functional transition program. This program is developed with the purpose of providing young adults with cognitive disabilities the opportunity to receive realistic vocational and functional life skills instruction to develop skills needed to successfully transition into a more independent life. There are three distinct programs at the Transition Academy to support students with varying transition outcome goals

GRADUATION

Transition Academy students participate in the Graduation Ceremony and related senior activities in the spring before they come to the Transition Academy. Diplomas will be awarded after the completion of the program, or upon the age of 22.

ENTRANCE

High school seniors who have completed four years of high school, participated in GAA, have completed academic diploma requirements, have had access to campus-based domestic, daily living, and recreational activities or access to weekly community based work experiences are eligible to participate in the Transition Academy Enrollment Process.

Reaching for Independence through Structured Environment (RISE)

RISE is a functional life skills program emphasizing community engagement.

Assisting Developing Adults with Pre-vocational Transition Opportunities (ADAPT)

ADAPT is a pre-vocational instruction program.

• Pre-Employment Skills

• Communication Skills

• Campus-based work learning experiences

• Self-advocacy Skills

• Independent Living Skills

 

 

Students with Opportunities for Achieving Regular Employment (SOAR)

SOAR is a vocational instruction program.

• Employability Skills

• Community Based Work Experiences

• Work Interpersonal Skills

• Independent Living Skills

• Self-Advocacy Skills
